How to request for the baptism of your child
- Complete a Baptism Registration. A fillable pdf document is provided in the Documents section.
- If you are not currently registered with St. John Neumann, a Parish Registration must also be completed. Baptizing your child here means we become the home of records for your child. All future sacraments will be recorded with us upon receipt of notification of sacraments celebrated at other parishes.
- Submit by email to [email protected] or deliver in person to the parish office the following: Baptism Registration, child's birth certificate, and Parish Registration.
- The parish will assign your baptism request to a Deacon.
- You will be contacted by the Deacon to schedule a pre-baptism class for both parents and godparents.
- The date of baptism will be set by you and the Deacon based on the availability of the church.
- Please include copies of the godparents sacrament certificates (baptism, confirmation) and if married, marriage certificate.
